SoviHep V – Hepatitis C Medicine
SoviHep V (Sofosbuvir 400 mg Plus Velpatasvir 100 mg) belongs to the class of anti-viral drugs used in Hepatitis C viral (HCV) infected patients with genotype 1 to 6. The fixed-dose combination (FDC) drug is known for explicitly inhibiting a specific type of protein that inhibits the multiplication (RNA replication) of cells thereby reducing the viral load.
The FDC drug (sofosbuvir + velpatasvir) imparts characteristics like,
- Treatment of almost all genotypes (1-6) of HCV infection
- A sustained virologic response (SVR) in between 95% to 100% after 12 weeks of treatment
- More than 90% of SVR in HCV patients with scarring of the liver (cirrhosis) irrespective of genotype
Administration of SoviHep V (sofosbuvir 400mg + velpatasvir 100mg):
SoviHep V (Sofosbuvir 400 mg Plus Velpatasvir 100 mg) is administered with or without food. It is taken in a form an FDC i.e. 1 tablet (sofosbuvir + velpatasvir combination) once daily for 12 weeks.
Suggested dosage of SoviHep V (sofosbuvir 400mg + velpatasvir 100mg) in HCV genotypes 1 to 6 patients:
- Without cirrhosis and cirrhosis with a functional system:
SoviHep V (sofosbuvir 400mg + velpatasvir 100mg).
- Cirrhosis without a functional system:
SoviHep V (sofosbuvir 400mg + velpatasvir 100mg) plus ribavirin.
Other properties of the FDC combination drug:
Once swallowed, sofosbuvir is absorbed within 30 minutes-2 hours and velpatasvir takes around 3 hours to completely absorb. Sofosbuvir has a binding affinity of 61%-65% whereas velpatasvir binds with 99% affinity. Once it binds with viral cells, sofosbuvir inhibits non-structural protein 5B (NS5B) whereas velpatasvir inhibits non-structural protein 5A (NS5A) that carries out the process of multiplication of viral cells. Once their activities are done, more than 50% of sofosbuvir and velpatasvir is expelled from the body within 40 minutes-27 hours and 15 hours respectively.
Note: The duration and frequency of dosage can be changed based on the interactions with other drugs and initial experience of the patients. It is mostly dependent on the doctor’s judgement
When should SoviHep V (sofosbuvir 400mg + velpatasvir 100mg) be avoided or used cautiously?
If patients are suffering from or had,
- History of Hepatitis B viral infection before taking sofosbuvir.
- Liver transplant before the initiation of dosage
- Complications in the liver other than HCV infection
- Heart problems
- Kidney disease or undergoing dialysis
- HIV infection
- Other severe medical condition
Issues during child-birth if patients,
- Are pregnant or planning to get pregnant
- Are male and your partner is planning to get pregnant
- Are breastfeeding or planning to breastfeed
Side-effects of SoviHep V (sofosbuvir 400mg + velpatasvir 100mg)
Most common side-effects:
- Headache
- Tiredness (Fatigue)
Note: To avoid or overcome the side effects, drink plenty of water and follow a balanced diet. If you are using any other medications or supplements, tell your doctor before taking the medicine.
Serious side-effects: Slow heart rate (bradycardia) with symptoms like,
- Shortness of breath (Dyspnea)
- Confusion
- Dizziness
- Chest pain
- Fainting
- Fever
- Problems in reminding
Side-effects in HCV patients without cirrhosis or cirrhosis with a functional system:
- Nausea
- Physical weakness (Asthenia)
- Lack of sleep (Insomnia)
- Headache
- Tiredness
Side-effects in HCV patients with cirrhosis and an unfunctional system:
- Diarrhea
- Low count of red blood cells (RBCs) (Anemia)
- Lack of sleep
- Headache
- Tiredness
- Nausea
Uncommon side-effects:
- Depression
- Severe rashes on the skin
